Our Employer Plan allows businesses to offer affordable, high-quality primary care as a benefit for their employees.

  • Employee Rate: $85/month per employee
  • Minimum Requirement: At least 5 employees to qualify
  • Enrollment Fee: One-time $200 per employer. Enrollment fee is per employer (not per employee), which means you pay just once and can add as many employees as you want under that single fee.
  • HSA Eligible: Payments can be made with Health Savings Accounts (HSA), allowing employers and employees to maximize pretax dollars
  • Tax-Friendly Options: In Ohio, employers may also be able to set up payments as pre-tax without an HSA by using a Health Reimbursement Arrangement (HRA), a Section 125 Cafeteria Plan, or by paying directly as a business health benefit (which is tax-deductible for the employer).
  • Flexibility: Employers can choose to cover all or part of the membership cost

Example 1: Employer Covers 100%

  • 10 employees
  • $85 × 10 = $850/month
    One-time enrollment = $200 (per employer, unlimited employees)
  • Total first-month cost = $1,050
  • Ongoing monthly cost = $850

Example 2: Employer Covers 50%, Employees Pay 50%

  • 10 employees
  • Employer pays $42.50 × 10 = $425/month
  • Employees each pay $42.50 = $425/month total from employees
  • One-time enrollment = $200
  • Total first-month employer cost = $625
  • Ongoing monthly employer cost = $425

  • One-time enrollment = $200

Employer Tax Treatment Options in Ohio

Health Reimbursement Arrangement (HRA):

  • Employers can reimburse employees for DPC membership fees through an HRA.
  • Reimbursements are tax-deductible for the employer and not taxable income to the employee.
  • This works well for covering the DPC membership as a defined health benefit.

Section 125 Cafeteria Plan:

  • Employers can allow employees to pay for DPC membership with pre-tax dollars via payroll deductions.
  • This requires a Section 125 plan setup, which is common for small-to-medium businesses.

Direct Employer Payment (Business Deduction):

  • If the employer pays DPC fees directly as part of an employee health benefit program, it is generally treated as a tax-deductible business expense.
  • The employee does not count it as taxable income.

QSEHRA (Qualified Small Employer HRA):

  • For small businesses (<50 employees), a QSEHRA allows the employer to reimburse employees tax-free for medical expenses, including DPC membership.
